Mobile Wellness RPN

Summary
 
Under the direction, accountability, and guidance of the Mobile Wellness Team Lead the Registered Practical Nurse (RPN) will deliver individualized or group, functional based care and support developed in collaboration with the Mobile Wellness multidisciplinary team. RPN will help clients improve their daily living skills, boost confidence, and foster social connections to promote independence and wellbeing. 
 
Working within an interdisciplinary team and guided by the Seven Grandfather Teachings and the Woven Blanket Model of Care, the Coordinator collaborates and consults with internal and external partners to facilitate referrals, coordinate care, and ensure service continuity. The Mobile Wellness Program provides specialized services to individuals experiencing serious mental illness; therefore, the RPN must possess an in-depth understanding of treatment pathways, crisis intervention, and recovery-oriented approaches.
 
Key Responsibilities
 
Direct client care
·        Understanding and implementing the treatment plan set out by the Mobile wellness multi-disciplinary team within scope of practice
·        Providing education and training to patients, family members, and caretakers
·        Providing detailed and accurate patient records Assisting client with activities of daily living
·        Collaborate with referring agencies to gather additional information and provide updates on referral status.
·        Participate in regular group consultation and case discussions with the Mental Health and Addictions team
 
Navigation
·        Coordinate and facilitate access to traditional healing, land-based, and other culturally relevant programming.
·        Support clients in accessing external health and social service programs
·        Maintain a comprehensive understanding of local resources and service pathways across Manitoulin Island.
 
Community and Program Development
·        Participate in the promotion of Mobile Wellness services through outreach, education, and community engagement activities.
·        Attend and contribute to community and organizational committees, working groups, and program planning initiatives as directed.
 
General Service Responsibilities
·        Complete all charting, documentation, and data entry within the Electronic Medical Record (EMR) system accurately and in a timely manner.
·        Compile and submit reports, statistics, and quality indicators required by funders and the organization.
·        Ensure strict adherence to privacy, confidentiality, and PHIPA/FIPPA standards.
·        Adhere to organizational policies, procedures, and professional standards of practice.
·        Complete all required leave documentation (e.g., annual leave, professional development, certified or uncertified sick leave, overtime, etc.) in a timely manner.
·        Participate in professional development, certification, and training as required by
·        Attend staff meetings, strategic planning sessions, wellness activities, annual general meetings, and other required organizational events.
·        Perform other duties as assigned.
 
Qualifications
 
Mandatory
·        Registered Practical Nurse registered in good standing with the College of Nurses of Ontario
·        Excellent interpersonal, communication, and problem solving skills
·        Valid Class G driver’s licence, access to a reliable vehicle, and a clear driver’s abstract.
·        Clear Vulnerable Sector Criminal Record Check.
 
Preferred
·        Experience working with First Nation communities and/or Indigenous organizations.
·        Familiarity with electronic medical record systems and data collection processes.
·        Knowledge of community resources and service providers within the Manitoulin Island region.
·        Ability to speak or understand Anishinaabemowin considered an asset.
·        Minimum of three (3) years of experience providing clinical or community-based services within a mental health and/or addictions setting
 
Work Conditions
·        Services are delivered across Manitoulin Island District.  
·        Frequent travel and occasional evening or weekend work may be required.
·        Combination of office-based, community-based, and mobile/outreach service environments.
·        Exposure to emotionally distressing situations may occur; self-care practices and debriefing supports are available.
