Summary

Under direction and supporting the Team Lead, the Traditional Intake/Care Coordinator is responsible for providing guidance and support to clients and families. This position will provide cultural and emotional support through the delivery of traditional, cultural and relevant health and wellness programming. Services are delivered within the seven First Nation communities and off reserve clientele. The Traditional Intake/Care Coordinator will assist in organizing the Eniigaaniijik Traditional Advisory Committee meetings.

Job Duties

1. Intake and Scheduling
• Receive client requests for traditional health and provide consultation, intakes, and referrals for traditional healing
• Refer clients to service provider
• Schedule clients with provider within Electronic Medical Records (EMR)
• Coordinate community visits with Cultural Supports
• Collaborate with referring person to receive further information, to update status of referral
• Schedule initial/follow up appointments for clients
• Monitor status of waitlist

2. Navigation
• Coordinate and facilitate access to internal support services (i.e. Primary Care, Mental Health & Addictions and Health & Wellness
• Refer clients to external service providers and programs
• Network with community agencies and supports

3. Perform administrative duties in support of the program’s goals and objectives
• Input accurate client information and other relevant data into Electronic Medical Records (EMR)
• Coordinate travel arrangements for Traditional Healers/Knowledge Keepers
• Engage in all activities related to organizing and supporting meetings, including scheduling, agenda, setting meeting details, minute taking and distribution of documents for the Eniigaaniijik Traditional Advisory Committee
• Complete monthly expense forms
• Assist in the development and implementation of the organization’s quality improvement initiatives

4. Perform other duties as assigned within the scope of the position

Requirements

Mandatory
• College diploma in office administration, social services, or related field
• Knowledge in logistics and scheduling
• Valid class “G” license and own reliable vehicle
• Clear driver’s abstract
• Clear Vulnerable Sector Check

Preferred
• Ability to speak, understand or willingness to learn the Anishinaabemowin language
• Previous experience working in a health-related organization
• Indigenous Cultural Safety Training
• Knowledge of Electronic Medical Records (EMR) database
• Current CPR Level C and First Aid Certification

Knowledge
• Understanding and sensitivity related to Anishnaabe language and culture
• Awareness of traditional healing practices
• Knowledge of First Nation communities of Manitoulin Island
• Knowledge of the cultural services available on the First Nations within the Manitoulin Island/Espanola catchment area
