Description
Summary
The Registered Social Worker- Older Adult Services (55+) specializes in supporting Anishinabek adults and their families. The social worker will provide direct mental health counseling, conduct psychosocial assessments, and navigate complex healthcare transitions, with a goal of maximizing clients' independence, dignity, and quality of life.
Key Responsibilities
· Assist the client in identifying care plan goals to ensure that the client’s psychosocial needs are met, enhancing their overall quality of life
· Provide individual counseling to help elders cope with grief, trauma, dementia, depression, and anxiety.
· Conduct psychosocial assessments including evaluating a client's cognitive function, mental health, living situation, and social determinants of health to determine appropriate care.
· Collaborate with social and health care services to ensure a wholistic approach to client care
· Refer to appropriate cultural, social and health care services
· Caregiver Support: Provide education and resources to prevent caregiver burnout
Requirements:
Mandatory
· Minimum bachelor’s degree in social work (BSW). Masters level preferred (MSW).
· Must be in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
· Criminal Record and Vulnerable Sector checks
Knowledge
· Strong knowledge of geriatric-specific assessment tools and understanding of age-related mental health challenges
· Knowledge of Substitute Decision-Making and capacity considerations
· Understanding and sensitivity for the Anishinaabe culture
Ability
Ability to speak Anishinaabemowin is an asset
Work Conditions
Frequent interruptions within the office environment
Frequent travel across the Manitoulin Island district required
